    Overview

    The 12 month fellowship includes non-cardiac thoracic surgery and related outpatient work-up, planning of operative approaches, and follow-up. There is exposure to a wide variety of endoscopic techniques, including rigid bronchoscopy, flexible bronchoscopy and fluorescence bronchoscopy with associated advanced laser techniques. The fellows are also involved in the management of complex endoscopic cases for a wide variety of malignant and benign esophageal disorders. This includes minimally invasive esophagectomy, laparoscopic gastric bypass, video-assisted thoracoscopic surgery (VATS), rigid and flexible esophagoscopy, placement of expandable metal stents and applications of photodynamic and YAG lasers. The fellow is generally the operating surgeon on most procedures and the faculty member is the teaching surgeon or first assistant. On occasion, the complexity of an operative case surpasses the experience and capabilities of the trainee and in these circumstances the fellow is assigned as the first assistant to the attending surgeon.

    A competitive salary is offered as well as the opportunity for moonlighting with additional compensation. The fellow will be expected to present at two national meetings. In addition, he or she will attend weekly morbidity and mortality conferences, esophageal conferences, departmental research conferences, and grand rounds.

    Procedural Requirements

    Fellows are expected to maintain an up-to-date log of their operative cases. Fellows will be able to show competency in advanced laparoscopic and thoracoscopic techniques and will have completed, with minimal support from attending, the following procedures:

    Fellows must complete a case log on a quarterly basis.

    Didactic Components

    There are several teaching conferences devoted to the clinical and research education of the trainees during their fellowship. Thoracic tumor board is a weekly conference that includes members of the thoracic Division, medical oncology, radiation oncology and pathology. This is an ideal opportunity for the trainees to understand the current limitations of therapy and the current status of clinical trials for various thoracic malignancies. The thoracic division conducts a separate morbidity and mortality conference weekly. Thoracic staff, fellows, CT residents and general surgery residents review the previous week’s morbidities and discuss management issues. There is another weekly conference conducted that is entirely devoted to complex benign esophageal disorders. This conference is again a multidisciplinary team coordinated by the thoracic division and includes members of the Gastroenterology Division and the General surgery divisions. In addition to these required conferences, there is a weekly research conference that includes members of the thoracic division, fellows, and CT residents. This conference reviews ongoing and possible future lab projects both at the basic science and clinical level. The conference is attended by the PhD's from the basic science lab. During the course of the fellows' experience at UPMC, many get involved in related research projects and present at the thoracic surgery national meetings.
